Summary of Employee Benefits

Medical Insurance

Our medical plan is offered through United Healthcare. We offer a high deductible health plan combined with an employer sponsored health savings account. A summary of our medical plan and available 2010/2011 employer automatic and matching dollar contributions to the health savings account are attached.

Dental Insurance

Bethesda offers dental insurance through Assurant Employee Benefits that includes an in network annual limit of $1,700 and 100% coverage on preventative care. Participants on the dental plan receive a Vision Discount Plan through VSP at no cost additional cost.
Payroll Deductions: The following chart provides the pay period premium deduction for medical and dental insurance benefits. Premiums are deducted from earnings on a pre-tax basis.

Life Insurance

Company-paid Basic Life and Accidental Death and Dismemberment Insurance equal to 1.50 times your base wages, rounded to the next higher 1,000, to a maximum amount of $100,000.

Supplemental Life Insurance

Fulltime employees may purchase Supplemental Term-Life Insurance combined with Accidental Death and Dismemberment Insurance at group rates for you, your spouse, and dependent children.

AFLAC Supplemental Benefits

Aflac offers supplemental benefit plans including a cancer plan, accident plan, specified event plan, and a vision plan. These are available through payroll deduction.

Flexible Savings Account (FSA)

The FSA plan year limit is $6000 for medical reimbursement and $5,000 for dependent care reimbursement.

403b Retirement Plan

Employees may enroll and begin participation immediately upon hire. After 1-year of service, Bethesda will offer a discretionary employer contribution for all eligible employees. Employer contributions are subject to a 5-year vesting schedule and may change as determined by Bethesda’s Board of Directors.

Please note: Most benefits illustrated are based on fulltime employment status and a 40-hour work week. Please refer to the employee manual for employees working less than a 40-hour work week. Insurance benefits are available the first of the month following 90-days of employment for qualified employees.
